Your First Visit

Thank you for choosing Heart of Florida Health Center as your new medical home.

Making an appointment is easy and convenient. We have one phone number that enables you to make appointments for all our services at all our locations, (352) 732-6599.

Please bring the following to your first appointment:
  • Driver license or ID card
  • Insured, insurance card
  • Uninsured and/or applying for the sliding fee scale, 1 month of most recent proof of household income

We want your experience to be as easy and comfortable as possible. If you have any questions, please feel free to call or email us.

 

Appointment Guidelines:

Patients must confirm, cancel, or reschedule by 3:00 p.m. the day before.

Arrival Time:

  • New patients: 30 minutes early for registration.
  • Established patients: 15 minutes early.
  • Late Arrivals may have to be rescheduled.
Patients with 3 no-shows and/or late cancellations in a six-month period will be limited to same-day appointments for the following six months.
