LPN
Job Summary |
The role of the Licensed Practical Nurse is to work as a member of the primary health care team in a supportive capacity to the Physician and Nurse Practitioners. |
Qualifications and Requirements |
· License: Current Florida LPN license and certification to administer medication.
· Education: Graduation from an accredited LPN program. · Ability to use the nursing process, as evidenced by physical and psychosocial observation skills, interviewing techniques, communication, and interpersonal skills, problem-solving, and clinical judgment. Maintain patient confidentiality. Spanish fluency is desirable, but not essential. · Confidentiality: · Demonstrates knowledge and full understanding of HIPPA patient privacy rights. Maintains confidentiality of all medical, financial, and other sensitive materials and information in printed, electronic, or verbal form, which may jeopardize the privacy of patients and/or employees. Accesses and uses the minimum necessary patient identifiable information to perform job responsibilities and duties and only for authorized purposes. · Work Environment: · The nurse works in a medical clinic environment, which is generally indoors in a climate-controlled environment. The working environment may change somewhat between locations. Nurses will be assigned a home location. However, location assignments will change from time to time, and the nurse must be able to cover at any location, as assigned, either temporarily or on a long-term basis.

Essential Functions |
· Greets patients and visitors in a friendly, professional manner.
· Understands that our patients are our priority and maintains a respectful, pleasant and polite demeanor at all times. · Resolves patient issues as appropriate – notifies appropriate supervisor if unresolved. · Measures and documents patient’s primary problems, including vital signs, basic labs, psycho-social history and medications in order to prepare the patient for examination by the provider, anticipating the need for required supplies and equipment. · Reports to the provider any significant social or other data obtained during conversation with patient or his/her family. · Performs general nursing care to patients. Administers prescribed medications and treatments in accordance with nursing standards of practice and established scoped of practice for licensed practical nurses and as prescribed by physician or nurse practitioner. · Maintains basic instruments and equipment. Performs technical functions as determined by the Chief Medical Officer. · Observes, records, and reports patient’s condition and reaction to drugs and treatments to physicians. · Attaches lab and radiology results to patient chart for provider review and notifies provider of test results outside abnormal parameters. · Recognizes signs/symptoms and/or patient changes and notifies Provider. · Inventories, orders and restocks supplies as needed. · Creates and documents in care plans and self-management support flow sheets · Returns patient calls/voicemails and places calls as requested by provider in a timely manner. Documents this process in the electronic medical record. · Provides patient education to clients utilizing the electronic medical record. · Works as a member of the care team to reviews medical records for completeness of data prior to the visit. · Tracks, records and scans radiological procedure orders for provider. · Actively participates in identification and determination of conditions to ensure high quality care to clients/patients and their families. · Attends required meetings and participates in health center work as requested. · Administers immunizations per CDC guidelines. · Conducts/participates in care team huddles daily for efficient communication among the team. · Participates in Population Health Management activities. · Performs phlebotomy/lab tests according to lab policy and procedure. · Maintains quality and aseptic techniques using standard policies and procedures. · Performs testing/procedures per provider orders such as EKGs, ear irrigation, dressings, ace wraps, nebulizer treatments. · Maintains quality and aseptic techniques using standard policies and procedures. · Performs quality control tests per established protocol. · Washes and prepares instruments for sterilization · Demonstrates priority setting and organizational skills. · Exhibits punctuality and dependability in attendance and work habits · Performs other related job duties as required

Dental Hygienist
JOB DESCRIPTION |
Job Summary |
Responsible for delivering dental hygiene services in accordance with Florida Statutes 466 and Administrative Rules 64B-5, including but not limited to providing dental care at HFHC locations and other off-site centers as needed (including school-based clinics and health fairs), with emphasis on prevention and education of patients about oral diseases. Responsible for performing the dental treatment and associated tasks with adherence to HIPAA, CDC, and OSHA standards. |
Qualifications and Requirements |
Education: · Graduate of a CODA-accredited dental hygiene program or internationally trained dentist with completed dental hygiene training and licensing requirements, including dental radiology certification, required. · Certification can be obtained within the dental hygiene program or post-graduate continuing education.
Certification and Licensure: · Must have passed the National Dental Hygiene Boards and possess and maintain a current and active Florida State Dental Hygiene Licenses (Department of Health/Division of Medical Quality Assurance); no history of license revocations or suspensions in any other state. · Current BLS certificate from an accredited AHA provider within 30 days from the date of hire – may be available from HFHC upon request.
Experience: · Minimum One (1) year of dental hygiene clinical experience preferred. · Experience working with children is a plus. · Experience dealing with underserved populations and cultural competency a plus. · Bilingual preferred – Spanish/English helpful. · Must have the ability to effectively use computer software and an electronic health record system. · Nitrous Oxide Analgesia training is preferred.
· Must be able to stay positive in stressful situations and deal professionally with unhappy, fearful, and confrontational patients. · Must maintain an effective and positive professional working relationship with all dental and non-dental staff at all times. · Must have general knowledge of basic dental policies & procedures and oral health issues, especially regarding oral hygiene. · Must be knowledgeable in dental terminology. · Must be competent in general office skills such as computers (Microsoft Office products), faxes, telephones, and copy machines. · Must be knowledgeable of office software (Office, Outlook, Excel), digital radiography software, and knowledge of EHR preferred. · Must be able to organize and prioritize workload. · Must maintain an effective and positive professional working relationship with staff at all times. · Must have the ability to understand and respond effectively and with sensitivity to special population groups. Special population groups include those defined by race, ethnicity, language, age, sex, sexual orientation, economic standing, & others. · Must have excellent people skills – good communication required. · Must have or be willing to train in child and adult behavior management. · Must have good manual dexterity for the utilization of specialized equipment. · Must be team-oriented and open to learning the front office aspect of dental care, including but not limited to insurance verification and scheduling.

We are in need of Medical Assistants to provide our patients with the highest level of care. As a valued part of our medical team, our MA’s support the doctors and nurse practitioners by providing quality primary care to adults and children.
Duties include pre-visit planning, maintaining equipment and supplies, vitals, taking medical history, documentation and processing referrals.
High School Diploma or GED required, plus CMA or RMA. At least one year of medical office, clinic, or hospital experience.
