Your First Visit
Thank you for choosing Heart of Florida Health Center as your new medical home.
Making an appointment is easy and convenient. We have one phone number that enables you to make appointments for all our services at all our locations, (352) 732-6599.
Please bring the following to your first appointment:
- Driver license or ID card
- Insured, insurance card
- Uninsured and/or applying for the sliding fee scale, 1 month of most recent proof of household income
- New patients: 30 minutes early for registration.
- Established patients: 15 minutes early.
- Late Arrivals may have to be rescheduled.
Language & Accessibility
We offer free interpreter services, including American Sign Language (ASL). Please let us know if you need help with communication or require any additional support to fully access our services. If you need assistance with transportation or other accommodation, contact your Care Team – we’re here to help.
We want your experience to be as easy and comfortable as possible. If you have any questions, please feel free to call or email us.
Appointment Guidelines:
Patients must confirm, cancel, or reschedule by 3:00 p.m. the day before.
Arrival Time:
Patients with 3 no-shows and/or late cancellations in a six-month period will be limited to same-day appointments for the following six months.

Need a Status Update on Your Medical Records Request?
If it has been more than 30 days since you submitted your request and you have not received your records, please call (866) 984-1399.
These requests are managed by our third-party partner, Verisma, who may be able to assist you directly through this number.
