Job Summary

The Primary Care Provider delivers preventive, chronic disease maintenance, and episodic health care to patients of Heart of Florida Health Center within the identified scope of primary care practice.  The Provider supports the mission and goals of the health center. Utilizes professional skills to diagnose and treat patients who present with both acute and chronic health problems. Ensures efficient and effective patient-centered, team-based care through collaborative teamwork, problem-solving, and communication.  Provides care consistent with medical best practices and the policies and procedures of the organization. Complies with standards of care and related laws and regulations, including HIPAA and OSHA regulations. Protects the dignity, privacy, and confidentiality of patients and their families.

Qualifications / Requirements

  • Master’s Degree,
  • Advanced Registered Nurse Practitioner
  • Board Certified
  • Current, valid Florida ARNP license

Demonstrates knowledge and understanding of patient privacy rights. Maintains confidentiality of all medical,
financial, and other sensitive materials and information in printed, electronic or verbal form, which may
jeopardize the privacy of patients and/or employees. Accesses and uses the minimum necessary patient
identifiable information to perform job responsibilities and duties and only for authorized purposes.

Essential Functions

  • Adheres to practice guidelines, policies, and procedures.
  • Assesses and develops treatment plans according to set standards and treatment regimens in collaboration with patient/parent/or significant other.
  • Remains knowledgeable and compliant with HFHC financial and treatment eligibility guidelines.
  • Treats patients and co-workers with acceptance and respect for cultural differences.
  • Collaborates with Medical Director, staff, and other service agencies.
  • Makes appropriate referrals and consultations.
  • Utilizes resources appropriately.
  • Documents subjective, objective, assessment accurately, timely, and completely into HFHC Electronic Medical Record.
  • Maintains current problem and medication list.
  • Meets productivity targets, demonstrating priority-setting and organizational skills.
  • Provides age-specific preventive and health maintenance care.
  • Provides age-specific anticipatory guidance, teaching self-care and health promotion.
  • Demonstrates team spirit.
  • Completes superbill with 90% accuracy.
  • Consistently documents disclosures of protected health information.
  • Achieves 90% in QA Chart Review by collaborating physicians.
  • Collaborates in a multi-disciplinary manner in planning and providing health care and patient education.
  • Creates an environment of acceptance and respect for cultural differences.
  • Assesses and identifies physical, social, psychological, and health education needs
  • Orders and interprets diagnostic tests Analyzes subjective and objective data to develop a treatment plan in collaboration with patient/parent/significant other.
  • Implements care based on the treatment plan; evaluates progress toward desired outcomes.
  • Consults with collaborating physician for the management of acute patient medical changes.
  • Maintains primary accountability to patients/acts as a patient advocate in all matters.
  • Adheres to practice guidelines, policies, and procedures.
  • Addresses patient/family educational needs; plans and implements teaching and evaluates outcomes.
  • Uses protocols to prescribe cost-effective treatment modalities and pharmacological therapies.
  • Participates in quality improvement through peer review and other projects as necessary.
  • Assists in the supervision of medical support staff.

Physical Requirements

  • Continuous use of corrected vision, depth perception, wide field of vision, and color. Also, the job will demand continuous use of olfactory, auditory, and tactile senses.
  • This position is very active and requires standing, walking, bending, kneeling, and stooping all day.
  • This employee must frequently lift and/or move items over 25 pounds.
  • Must be able to lift, carry and handle equipment, supplies, and other work site materials according to position requirements.
