Heart of Florida Health Center Earns National Recognition for Patient-Centered Care

NCQA Patient-Centered Medical Home standards emphasize enhanced care through patient-clinician partnership

Ocala, FL, March 13, 2017 – The National Committee for Quality Assurance (NCQA) announced this week that Heart of Florida Health Center (HFHC) has received the highest level (Level III) of NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long-term, participative relationships.

The NCQA PCMH is a model of primary care that combines teamwork and information technology to improve care, improve patient experience, and reduce costs. Research shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care.

“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology, and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane.

This distinguished earned recognition, which is valid for 3 years, is aligned with joint principles of the PCMH established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics, and the American Osteopathic Association.

“This was an enormous undertaking that speaks to HFHC’s absolute commitment and dedication to providing high quality healthcare to all residents of Marion County,” said CEO Jamie Ulmer.
