PRESS RELEASE: Heart of Florida Health Center Earns National Recognition for Patient-Centered Care

NCQA Patient-Centered Medical Home standards emphasize enhanced care through patient-clinician partnership

Ocala, FL, March 21, 2022 – The National Committee for Quality Assurance (NCQA) announced this month that Heart of Florida Health Center (HFHC) has received the NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long-term, participative relationships.  HFHC has been awarded this quality distinction for the last 8 years.

The NCQA PCMH is a model of primary care that combines teamwork and information technology to improve care, improve patient experience, and reduce costs.  Research shows that medical homes can lead to higher quality and lower costs and can improve patient and provider reported experiences of care.

This distinguished earned recognition, which is valid for 3 years, is aligned with joint principles of the PCMH established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics, and the American Osteopathic Association.

“During the last couple of years our staff have exemplified the PCMH model. Being recognized and awarded again this year, speaks to HFHC’s absolute commitment and dedication to providing high quality healthcare to all residents of Marion County,” said CEO Jamie Ulmer.
