RN – Quality Team
The nurse in this position is a key member of the quality team, working to improve preventive care and outcomes for patients of our federally qualified health center. You'll engage patients and provide identified preventive care services needed. This may include, but is not limited to, health screenings, immunizations, education, and linkage with community resources to address social determinants of health. Patients may be scheduled with you directly or seen as a "warm handoff" from a provider during a primary care appointment. Travel between HFHC clinics in Marion County is required. Some services will be provided through telehealth. You'll also assist with data collection and analysis for UDS, HEDIS, and other quality of care metrics, and collaborate with providers on special projects to improve work processes and enhance patient and staff satisfaction. Current active Florida RN and three years of experience in primary care or similar healthcare environment is required. Previous experience in a federally qualified health center and with rapid cycle change processes is preferred. Family Medicine Physician – Core Faculty
Seeking core faculty member(s) who will be responsible for teaching, supervision, and formal evaluation of residents. Medical Director- Marion County Jail
Heart of Florida seeks a licensed medical doctor (current and clear,) qualified as required by all federal, state, and local laws, statutes, ordinances, rule, regulations, and codes. The Medical Director is responsible for assuring the quality, continuity, and appropriateness of health care services at Heart of Florida Health Center and the Marion County Inmate Health services. Clinical/Administrative: The Medical Director will provide clinical and administrative leadership, oversight, and coordination for all provider staff and will assure that patient care is efficient, effective, and evidence-based and delivered in a caring, competent, and culturally appropriate manner. The position requires significant clinical practice as well as completing administrative functions. The Medical Director supervises all physicians and contributes to the strategic direction and decision making of the organization and supports the mission, strategy, values, and goals of our health center. Director of Nursing-Marion County Jail
Under the administrative direction of the Health Services Administrator, Director of Nursing manages and coordinates nursing, patient care and associated operational services within the Marion County Jail. Formulates clinical and patient care policies, protocol and procedures, establishes and manages budgets and business/administrative systems, and participates in strategic decision making for the operation. Directs, coordinates and oversees the administration of all associated nursing and support staff. The practice manager is responsible for work-flow, performance, and the overall management of the day to day processes for the assigned clinic operations, with the goal of excellent patient service and physician/staff productivity. Our customer associates are the first person our patients see when they arrive and the last when they check out, so are a valued part of the care team. Customer Associates greet patients on arrival, review patient information and enter corrections or updates, accept payments, and schedule return visits. Customer Associates also answer clinic phones and help resolve patient questions and issues. Minimum requirements: High School Diploma or GED; three years of customer service experience, medical practice setting preferred; organized, able to multi-task, excellent verbal and written communication skills.
