Helping Our Patients Get The Care They Need

Case Managers are critical to ensuring the needs of all patients and are addressed within the care setting so that quality of life, care progression, and meeting the social determinant of health are maximized. Case management practice is essential to improving our patient’s experience. Our Case Managers are healthcare professionals who serve as patient advocates to support, guide, and coordinate care for patients, families, and caregivers as they navigate their health and wellness journeys.

 

Case Management Services Include:

  • Needs assessment
  • Referrals to local and national services and resources
  • Care coordination. Coordinating appointments and scheduling transportation
  • Assist with locating low cost/no cost durable medical equipment, such as wheelchairs, walkers, and canes.
  • Through our Outreach program, Case Managers educate our community about the many services we provide.
